Após desmaiar, a primeira coisa que Jack (Matthew Fox, “Party of Five”) sente é dor. Depois, o sol queimando o seu rosto. Uma floresta de bambu. Fumaça. Gritos. Logo ele chega à terrível descoberta de que o avião em que ele estava se partiu no ar e caiu numa ilha do Pacífico. Logo seus instintos de médico lhe dizem que as pessoas estão precisando de ajuda.
Perdidos, os 48 sobreviventes tentam pegar o que for possível para a sobrevivência deles entre os destroços do avião. Muito pânico. Muitos guardam suas últimas esperanças enquanto aguardam o resgate. Outros descobrem novas forças que eles nem imaginavam que tinham — como Kate (Evangeline Lilly), que, sem nenhum treinamento médico, logo se vê costurando os ferimentos do médico. Hurley (Jorge Garcia) — um homem com um ingênuo senso de humor apesar da situação desesperadora — faz o possível para manter a calma e ajudar as pessoas próximas a sobreviver. Charlie (Dominic Monaghan, “O Senhor dos Anéis”) é um astro do rock em decadência que esconde um doloroso segredo. Sayid (Naveen Andrews, “O Paciente Inglês”) é um homem do oriente médio que terá que lutar contra o preconceito racial demonstrado por alguns de seus colegas sobreviventes.
Jin (Daniel Dae Kim, “Angel”, “24 Horas”) e Sun (Yunjin Kim) são um casal de coreanos que, devido às suas tradições, valores e língua, enfrentam ainda mais dificuldades para se comunicar com os demais. Sawyer (Josh Holloway) tem um certo ar de perigo que ronda sua personalidade, e seu intenso senso de desconfiar de todos ao seu redor poderá provar ser algo fatal para seus colegas náufragos. Michael (Harold Perrineau, “Matrix Reloaded”, “Matrix Revolutions”) acaba de ganhar a custódia de seu filho de 9 anos, Walt (Malcolm David Kelley, “Antwone Fisher”), após a morte de sua ex-mulher — eles são pai e filho que nem ao menos se conhecem. Locke (Terry O’Quinn, “Alias”, “Arquivo X”) é um misterioso e retraído homem que guarda uma grande ligação com a ilha, maior do que qualquer outro sobrevivente. A egoísta Shannon (Maggie Grace, “Oliver Beene”) — que decide pintar suas unhas do pé em pleno caos — e seu afastado e controlador irmão Boone (Ian Somerhalder, “Smallville”) — que constantemente está brigando com ela e que terá que aprender a viver com os demais caso eles queiram sobreviver. E também há a jovem Claire (Emilie de Ravin, “Roswell”), uma mulher que está grávida de oito meses e que não está preparada para as dificuldades da maternidade — especialmente numa ilha deserta.
Esse grupo de amigos, familiares, inimigos e estranhos terão que trabalhar juntos contra o cruel tempo e o terrível local, caso eles queriam continuar vivos. Mas a ilha guarda muitos segredos, incluindo os intensos uivos das criaturas escondidas na floresta, que irá aumentar ainda mais o medo dos sobreviventes. Felizmente, graças à calma liderança de Jack e à equilibrada Kate, eles têm alguma esperança. Mas até mesmo os heróis têm segredos, como os sobreviventes irão descobrir.
